Surgical Pathology and related Prostate Needle Biopsy

Effective January 1, 2015, HCPCS prostate needle biopsy pathology codes G0417, G0418, and G0419 have been deleted and the definition of HCPCS code G0416 – surgical pathology, gross and microscopic examinations, for prostate needle biopsy, any method has been revised to remove the reference to 10-20 specimens. The Anthem BCBS policy dated January 1, 2015 reflects these changes.
