Additional Lab Codes Billed With Modifier QW

Effective for dates of service on or after September 1, 2009, laboratory CPT-4 codes 80069 (renal function test) and 82550 (creatinine kinase [CK], [CPK]; total) may be billed with a modifier QW (CLIA-waived tests). The QW modifier certifies the provider is testing for the procedure with the use of a CLIA-approved test kit that is considered to be a CLIA-waived test by CMS.
