2013 Molecular Pathology Fee Schedule

Cahaba has determined a gapfill allowance for the 2013 Molecular Pathology CPT codes. They can be obtained by doing a Search for a Single Code on the 2013 Clinical Lab Fee Schedule.

CHANGE NOTIFICATION TO REIMBURSEMENT POLICIES

For claims with for dates of service on or after May 20, 2013, Travel allowance services for collection of a Lab Specimen reported with HCPCS codes P9603 and P9604 are not eligible for separate reimbursement even when reported with modifier 59. The Laboratory and Venipuncture Services reimbursement policy will also be updated to reflect this change.

Drug Screening Services

BCBSGA has a new reimbursement policy, Drug Screening Services, regarding the reporting of Qualitative Drug Screen Testing, which will be effective with claims processed on or after December 8, 2012. They have adopted the CMS guidelines and will no longer reimburse CPTĀ© codes 80100, 80101, and 80104. Qualitative drug screening will now only be reimbursable using codes G0431 and G0434. Both codes G0431 and G0434 will be eligible for 1 unit of reimbursement per date of service.

CORRECT USAGE OF MODIFIER 59

Due to recent provider inquiries regarding incorrect denials of CPT codes billed with modifier 59, Cahaba ran a report to identify all claims denied as duplicates with the use of modifier 59. The report is being researched and adjustments will be made to incorrectly denied claims using an automated system. Until the claims have all been adjusted, our Clerical Error Reopening lines will not adjust claims denied as a duplicate appended with modifier 59 to avoid creating overpayments.
